This is a typical question answering screen showing the position of the qustion in the evaluation on the left of the screen and the full question and guidance on the right.  There are other text fields where you can enter notes, comments and recommendations.  A user would select the appropriate answer from the options - this question is a Yes/No question but there are many other types available.  N/A is 'not applicable'.
